Intake forms

Build your health questionnaires once and they go out automatically when a client books — no chasing, no paper forms to scan. You choose how often each form is sent: once per client, with every booking, or again after a set number of days. Clients complete them on their phone before they arrive, with no account or app required.

Create multiple forms and assign each one to a specific service or location. A deep tissue client gets asked different questions to a hot stone client. The most specific matching form is always sent — service-specific beats location-specific beats the default.

Not sure a prospective client is right for you yet? Open their record and send the form before any booking — with an editable note explaining why — to check suitability or contraindications before you confirm anything.

Teaching a class or treating a group? Create one shareable link and send it to everyone at once. Each person fills in their own form on their phone without needing a booking, an account, or a link of their own. If their mobile number matches someone already on your client list the form lands on that record; if not, a new client is created for you. Nothing on an existing record is ever changed by someone filling in a shared link. Turn the link off when you are done, or replace it if it has travelled further than you meant.

Any completed form can be downloaded as a PDF, from the responses page or straight from the client record — for your own files, an insurance query, or handing a client over to another therapist. Every question appears in the order you set it, and anything left blank reads "Not answered" rather than being dropped, so a gap in a health history stays visible.

All responses are encrypted at rest (AES-256) and kept private to your practice. Linked directly to the client profile, so you always have the right information to hand before a session.
